Shared Residency and Guardianship Affidavits
The shared residency affidavit is needed when the family is living with someone else and therefore cannot produce proof of residency with the guardian’s information on it. If you are unable to provide sufficient proof of residency because you are currently living with someone who is the actual Crowley ISD resident and there is no electric, gas, water bill or lease agreement in your name, the CISD resident will need to complete an Affidavit for Shared Residency stating that you live in his/her home. This form is available here on the CISD website, at your campus and at the CISD Welcome Center. The Shared Residency form must be notarized by an approved CISD employee at the Welcome Center, Admin Annex or your home campus and the completed form must be submitted with all other enrollment documents at the campus of assignment or online. Upon approval at the campus level, you will be allowed to proceed with the enrollment process.
The guardianship affidavit is needed when the students are in the care of someone other than the individuals listed on the birth certificate or any other legal documentation indicating them as the guardian.
